Do I need Notion?
No. Everything prints and everything is editable. Notion is offered because the markdown pastes in cleanly and the tables convert automatically.
Are these HIPAA compliant?
No template can be. Compliance depends on how you store, transmit and handle records, not on the document itself. These are administrative and clinical templates you adapt.
Which note format should I use?
The one you will actually finish. SOAP is the most widely recognised, DAP is faster for talk therapy, BIRP makes the intervention explicit and is common in agency and group settings. All three are included.
Are any assessment instruments included?
No copyrighted instrument is reproduced. Common measures are named with a score field so you can record results from a tool you are licensed to use.
Is it state-specific?
No, and no template safely can be. Consent, retention and documentation requirements vary by state and licensing board, and the documents say so where it matters.
